Job summary

The Lakes Home Care Services is seeking a dedicated Physical Therapist Assistant to join our team providing in-home therapy. You will assist Physical Therapists with treatment plans, help patients regain movement, and educate families on exercises and care routines.

Ideal candidates have experience in geriatrics, orthopedics, or hospital settings and are comfortable documenting progress and coordinating with other healthcare professionals.

Qualifications

• Experience in patient care, geriatrics, orthopedics, or hospital/home care settings.
• Knowledge of physiology and patient movement.
• Familiarity with inpatient therapy procedures.

Responsibilities

• Provide assistance to Physical Therapists in developing and implementing treatment plans.
• Work with patients to help them regain movement and manage pain.
• Monitor and document patient progress during therapy sessions.
• Educate patients and their families on at-home exercises and care routines.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
