Qualifications

• Licensed PT in Utah or eligible per state requirements.
• CAPTE-approved education program or substantially equivalent education.
• Credentials evaluation may determine equivalency if trained outside the United States.

Responsibilities

• Assists physician or care provider in evaluating level of function and helps develop the plan of care.
• Advises and consults with family and agency personnel.
• Participates in in-service programs and presents as assigned.
• Understands and adheres to established policies and procedures.
• Participates with health care personnel in patient care planning.
• Directs PTA and HHA personnel per requirements.
• Completes initial history and evaluation, notifies physician, and submits plan of care for approval.
• Performs skilled physical therapy procedures as ordered by physician.
• Consults with physicians regarding changes in treatment.
• Writes progress reports to physicians.
• Instructs patients and family in home programs and activities of daily living.
• Attends staff meetings and patient care conferences.
• Supervises PTA every 30 days and HHA as assigned.
• Completes and submits OASIS assessments and other required documentation.
• Uses ICD-10 diagnoses and codes appropriately.
• Completes discharge summary upon completion of therapy services.
• Prepares 60-day summaries for continued treatment when required.
• Observes confidentiality and safeguards patient information.
• Maintains timely documentation and records.
• Demonstrates sound judgment and time management.
• Maintains a clean and professional work environment.
• Maintains CPR certifications.
• Performs other duties as assigned.

Education

Licensed Physical Therapist (Utah)

CAPTE-processed education or equivalency

Credentials evaluation equivalency (if trained outside US)
