Job summary

Amazing Care Home Health Services in Ogden, UT is seeking a Part-Time Physical Therapist to provide in-home care, evaluating and treating clients across ages in their homes and community settings.

You will implement treatment plans, collaborate with physicians, and coordinate with caregivers to improve cognitive, neurological, physical, and sensory functioning while supporting independence and quality of life. Mileage provided, 10-20 visits weekly.

Qualifications

• Doctoral degree in physical therapy is required.
• Utah PT license and active BLS certification preferred.
• Valid driver's license and auto insurance needed.

Responsibilities

• Provides evaluations and develops a care plan with skilled interventions.
• Implements PT treatment plans in home and community settings.
• Collaborates with physicians and care teams to ensure safe care.
• Instructs clients and families on the treatment plan as indicated.
• Documents per organization policy and maintains competence.
