Job summary
Amazing Care Home Health Services is seeking a part-time Physical Therapist to join our Logan, Utah team. The PT will implement treatment plans in home and community settings to assist clients across ages with cognitive, neurological, physical, and/or sensory functioning.
Responsibilities include evaluations, developing plans of care with physician collaboration, and educating clients and families. A valid Utah PT license and active BLS are required, with mileage reimbursement for patient visits.
Qualifications
- Doctoral degree in physical therapy required.
- Utah PT license required.
- Active BLS certification required.
- Valid driverâs license and auto insurance.
- Minimum 1 year clinical experience preferred.
Responsibilities
- Provide evaluations and develop plans of care with skilled interventions.
- Help pediatric and adult patients improve cognitive, neurological, physical, and sensory functions.
- Coordinate care with physicians and other health providers.
- Instruct patients and caregivers on treatment plans as indicated.
- Identify needs for additional services and refer as appropriate.
- Prepare and submit documentation per policy.
- Maintain clinical competency in physical therapy.
