Qualifications

• Must be licensed in the state where practicing, or meet CAPTE-equivalency requirements.
• Graduated from a CAPTE-accredited physical therapy program or equivalent.
• Two years of appropriate physical therapy experience preferred.

Responsibilities

• Assist physician or provider in evaluating function and develop the plan of care.
• Advise and consult with family and agency personnel.
• Participate in in-service programs and present as assigned.
• Follow established policies and procedures and participate in care planning.
• Supervise PTA and HHA per requirements and as assigned.
• Complete history/evaluation, notify physician, and obtain plan of care approval.
