Job summary

The Lakes Home Care Services in Florida is seeking a dedicated Physical Therapist Assistant to join our patient-centered team. You will assist Physical Therapists in creating and executing individualized treatment plans for clients receiving in-home care.

Responsibilities include helping patients regain movement, reducing pain, documenting progress, educating families on home exercises, and coordinating with healthcare professionals to ensure high-quality, compassionate care in the home setting.

Qualifications

• Previous experience in patient care, particularly in geriatrics, orthopedics, or hospital settings, is preferred or home care
• Knowledge of physiology and occupational health practices
• Familiarity with inpatient therapy procedures

Responsibilities

• Provide assistance to Physical Therapists in developing and implementing treatment plans
• Work with patients to help them regain movement and manage pain
• Monitor and document patient progress during therapy sessions
• Educate patients and their families on at-home exercises and care routines
• Collaborate with healthcare professionals to ensure comprehensive patient care
