You will monitor progress, educate patients and families on home exercises, and collaborate with healthcare professionals to ensure comprehensive care in a home-based setting.

Qualifications

• Previous experience in patient care, particularly in geriatrics, orthopedics, or hospital settings, is preferred or home care.
• Knowledge of physiology and occupational health practices
• Familiarity with inpatient therapy procedures

Responsibilities

• Provide assistance to Physical Therapists in developing and implementing treatment plans
• Work with patients to help them regain movement and manage pain
• Monitor and document patient progress during therapy sessions
• Educate patients and their families on at-home exercises and care routines
• Collaborate with healthcare professionals to ensure comprehensive patient care
