Job summary

Qualifications

• High school diploma or GED.
• Ability to maintain yearly training (HIPAA, OSHA, Mandatory Reporter).

Responsibilities

• Provide direct services to Consumers, including nights, weekends, and holidays.
• Assist Consumers with daily living skills and community participation to increase independence.
• Implement Individual Support Plans and document HCBS 20 Outcomes each shift.
• Maintain accurate documentation and submit timesheets on time.
• Attend monthly staff meetings/trainings and communicate with the team.
