Job summary

Angel Care Hospice and Palliative is seeking experienced Hospice Aides (CHHAs) to provide compassionate care for patients in their own homes, assisted living facilities, or nursing homes across Southern California.

The CHHAs will maintain flexible schedules, coordinate visits with patients and caregivers, and work with RN Case Managers and office staff to ensure high-quality care and support during difficult times.

Qualifications

• Current CHHA Certification - Required.
• Proven work experience in the healthcare industry
• Effective communication skills.
• Excellent organizational and time management skills.
• Problem solving skills and ability to multi-task.
• Compassionate and caring.
• CPR Certified.

Responsibilities

• Coordinate with assigned patients to schedule visits at the ordered frequencies.
• Communicate with the hospice team regarding patient needs.
• Report on patient status using a team-centric messaging system.
• Develop effective working relations and cooperate with hospice team.
• Take the extra initiative and interact with patients and their families to ensure satisfaction.
• Document all visits timely and with complete accuracy.
• Adhere to professional standards as outlined by protocols, rules and regulations
