Job summary

Parentis Hospice and Health is seeking a Certified Home Health Aide (Hospice) to provide compassionate in-home care in the Los Angeles area. The role supports daily activities, maintains comfort and dignity at end-of-life, and works with a multidisciplinary team to deliver personalized hospice care.

The aide will assist with ADLs, monitor patient status, offer emotional support, and document care activities.

Qualifications

• High school diploma or equivalent.
• Completion of a state-approved Certified Home Health Aide training program.
• Minimum of 1 year of experience working as a Certified Home Health Aide, preferably in a hospice setting.
• Experience in providing compassionate care to patients with terminal illnesses.

Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility.
• Monitor and report changes in the patient's condition to the hospice team promptly.
• Provide emotional support and companionship to patients and their families.
• Administer prescribed medications under the supervision of a registered nurse.
• Maintain accurate and timely documentation of patient care activities.
• Ensure a safe and clean environment for patients by adhering to infection control protocols.
• Collaborate with healthcare professionals to develop and implement individualized care plans.
• Educate patients and families on hospice philosophy and care practices.
• Respect patient confidentiality and uphold ethical standards in all interactions.
