Cabrillo Hospice in Poway, CA is seeking a caring Home Health Aide (CHHA/CNA) to support patients in their homes. You will provide personal care and companionship, following individualized care plans to enhance comfort and dignity.

This flexible full-time role offers meaningful, heart-centered work with a supportive care team. Active CHHA and CNA licenses are required; hospice/home health experience is preferred.

Qualifications

• Active CHHA and CNA licenses.
• Hospice/home health experience preferred.
• Compassionate, patient demeanor.
• Strong communication and teamwork.

Responsabilités

• Provide personal care: bathing, dressing, and mobility support.
• Ensure comfort and safety while offering emotional companionship.
• Monitor and report changes in patient condition to the care team.
• Assist with basic health needs per individualized care plans.
• Teach family members simple caregiving techniques.
