Job summary

Interim HealthCare of Mercer County is seeking a Home Health Registered Nurse (RN) to provide compassionate, patient-centered care in patients' homes across Monmouth and Ocean Counties.

You’ll assess, educate and coordinate care with a team of physicians, LPNs and CHHAs, while customizing plans of care and ensuring regulatory compliance.

The role offers flexible scheduling and work-life balance for RNs with NJ licensure and a minimum of two years of nursing experience.

Qualifications

• NJ licensed Registered Nurse (RN) with active NJ license.
• Minimum of 2 years nursing experience, preferably in home health.
• Bachelor's Degree in Nursing preferred.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Strong communication skills and compassion in patient care.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases.
• Coordinate with physicians and oversee LPNs and CHHAs providing patient care.
• Conduct patient assessments, document progress and report changes to the physician.
• Educate patients and family members on plan of care, medications, therapies, diet and exercise.
• Coordinate discharge planning and ensure goals are met.
