Job summary

National Recruiters is seeking experienced Registered Nurses for in-home health assessments across New Jersey. You will travel to patients’ homes to perform comprehensive health evaluations, documenting findings in the EMR and coordinating with the care team.

Ideal candidates have 3+ years of clinical experience and a NJ RN license, with strong communication and independent-work skills. Multiple NJ counties are open; expect flexible hours with a minimum commitment of ~50 hours per month.

Qualifications

• Active, unrestricted New Jersey RN license.
• Minimum of 3 years of clinical experience.
• Strong patient assessment and clinical judgment skills.
• Ability to work independently in patients' homes.
• Excellent communication and interpersonal skills.

Responsibilities

• Visit patients in their homes for comprehensive health assessments.
• Review medical history and current health conditions; reconcile medications.
• Perform review of systems and obtain/document vital signs.
• Conduct comprehensive physical assessment and document findings in EMR.
• Identify current/historical diagnoses and potential health concerns.
• Communicate professionally with patients and coordinate with the care team.
