Job summary

Adara Home Health, Inc. is seeking a Clinical Admissions Nurse to deliver 1:1 client care in patients' homes across White Bear Lake area.

This full-time role includes travel to client homes (1–3 hours per client) with an on-call rotation, reporting to the Clinical Supervisor, and serving Blaine, MN office coverage. You will assess needs, develop care plans, coordinate care with families and professionals, and maintain records while upholding company policies.

Qualifications

• Currently licensed as an RN in Minnesota.
• Minimum of 2 years experience in healthcare facility or home care.
• Strong organizational and independent work skills.
• Good written and oral communication; teaching skills.
• Understanding of Medicare certification and OASIS documentation.

Responsibilities

• Performs initial and periodic client assessments and updates care plans.
• Provides total client care including medications and treatments as ordered.
• Maintains client records and documents progress and outcomes.
• Coordinates client care with family, other caregivers, and resources.
• Ensures infection control and safety in the home environment.
• Participates in after-hours on-call process to ensure client care.
