Job summary

BAYADA Home Health Care, Inc. is seeking Registered Nurses (RNs) to support patients in the Upper Montgomery County area with flexible Full Time, Part Time, and Per Diem schedules. Join a team that emphasizes 1:1 care, work/life balance, and values-driven service.

You will provide home health care, follow care plans, document activities, and collaborate with clinical managers. A PA RN license and one year of clinical experience are required; travel to cases is expected.

Qualifications

PA RN license required.
Minimum of one year of recent clinical (medical/surgical) experience.
Home care experience preferred but not required.
Graduate from an accredited nursing program.
Ability to work independently and manage time effectively.
Strong interpersonal skills.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.

Responsibilities

Follow a designated care plan in accordance with patient's needs.
Make home visits to clients in designated geographic territories.
Administer medication, wound care, treatments, and procedures.
Monitor clients' conditions; report changes to Clinical or Client Services Manager.
Follow up with, execute, and properly document doctors' orders.
Perform client assessments as necessary.
Case management and coordination.
Accurately document observations, interventions, and evaluations in EMR.
