OakMed Home Nursing & Rehabilitation in Grand Rapids, MI is seeking a licensed Physical Therapist to provide home-based therapy. We offer flexible scheduling, competitive per-visit pay, and one-on-one patient care across West Michigan.

PTs will evaluate, treat, and document plans of care, traveling to patient homes. Cases include automobile accidents and workers’ compensation patients, with opportunities to build a flexible caseload.

Qualifications

• Current, unrestricted Physical Therapist license in Michigan.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation and ability to travel to assigned patient homes.
• Strong communication, organization, and clinical documentation skills.
• Ability to work independently while coordinating care with the appropriate healthcare team.

Responsabilités

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide skilled physical therapy interventions based on each patient’s needs and established plan of care.
• Monitor patient progress and adjust treatment as appropriate.
• Complete required clinical documentation accurately and in a timely manner.
• Communicate with patients, families, and other members of the healthcare team as appropriate.
• Follow applicable professional standards, licensing requirements, and organizational policies.
