Job summary

OakMed Home Nursing & Rehabilitation is seeking licensed Physical Therapists to provide home-based therapy across West Michigan. Flexible scheduling and per-visit pay allow clinicians to tailor a caseload close to home while delivering one-on-one patient care.

Qualifications

• Current, unrestricted Physical Therapist license in Michigan.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation and ability to travel to assigned patient homes.
• Strong communication, organization, and clinical documentation skills.
• Ability to work independently while coordinating care with the appropriate healthcare team.

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide skilled physical therapy interventions based on patient needs and plan of care.
• Monitor patient progress and adjust treatment as appropriate.
• Complete required clinical documentation accurately and timely.
• Communicate with patients, families, and healthcare team as appropriate.
• Follow professional standards, licensing requirements, and organizational policies.
