OakMed Home Nursing & Rehabilitation is seeking licensed Physical Therapist Assistants (PTAs) for home health opportunities in Grand Rapids, MI. You will provide one-on-one PTA care in patients’ homes under a supervising Physical Therapist, helping improve mobility and independence.

Formación

• Graduate of an accredited Physical Therapist Assistant program.
• Current, valid Michigan PTA license.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Provide PTA home visits under supervising therapist’s plan of care.
• Assist patients with exercises, mobility, and functional activities.
• Monitor patient response and report progress to the PT.
• Document visits accurately and timely.
• Educate patients and caregivers on prescribed routines and safety.
• Collaborate with clinical team to meet patient goals.
