Concierge Home Care in Brandon, FL is seeking a Home Health Occupational Therapist to deliver skilled therapy in patients’ homes, helping them regain independence and daily function. You will evaluate patients, develop tailored treatment plans, and collaborate with physicians and the care team to optimize outcomes.

Qualifications

• Active, unencumbered Florida Occupational Therapist license.
• Valid driver’s license and reliable transportation.
• CPR certification.
• Ability to supervise COTAs.
• Home health experience preferred but not required.

Responsibilities

• Conduct patient evaluations and home environment assessments.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on improving functional mobility and daily living skills.
• Educate patients and caregivers on adaptive strategies and safety techniques.
• Collaborate with physicians and interdisciplinary care teams.
• Supervise and delegate care to Certified Occupational Therapy Assistants (COTAs) when appropriate.
• Document visits efficiently using AI-assisted tools.
