Job summary

FeldCare Connects is seeking a self-motivated Certified Occupational Therapy Assistant to provide high-quality home health care in the Canyon Country area as an Independent Contractor. The role focuses on patient care relating to activities of daily living, functional status, and home safety for all ages.

The ideal candidate holds a California COTA license, has experience delivering home-based therapy, and can work flexibly in patients' homes.

Qualifications

• Graduation from an accredited Certified Occupational Therapy Assistant program.
• California COTA license.
• Ability and enthusiasm in working with all ages.
• Bilingual is a plus.

Responsibilities

• Provide services defined by the plan of care and coordinate with the health care team.
• Assist patients with activities of daily living and functional tasks.
• Perform home assessments and recommend adaptive equipment as needed.
• Monitor and report patient progress and responses to treatment.
