Job summary

Caretenders, part of LHC Group and the Optum family, is seeking an Occupational Therapist to support patients in the Lexington area within Home Health. You will assess and address functional status, ADLs, fine motor skills, and home safety.

You’ll work with a care team, follow state scope of practice, and use evidence-based techniques. A Kentucky OT license and a valid driver's license are required; CPR is preferred.

Qualifications

• Current Occupational Therapist license in Kentucky.
• Current driver's license, reliable transportation, and vehicle insurance.

Responsibilities

• Provides OT services within state scope, following the plan of care with evidence-based techniques.
• Evaluates functional status and CMS-relevant measures; collaborates with physician on care plan.
• Records patient responses to treatment and notes changes to condition for nurses/physician.
• Performs assessments of function using diagnostic and prognostic tests.
• Capable of home visits with diverse patients regardless of age, race, or condition.
