Drive time may be available, and the position is on-site at our Long Term Care facility in Fort Wayne. You will complete clinical nutrition assessments, document using the Nutrition Care Process in the EMR, and collaborate with the interdisciplinary care plan team.

Qualifications

• CDR credential and active dietitian licensure in Indiana.
• Excellent communication and collaboration with interdisciplinary teams.
• Client-centered approach with a positive attitude.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
