Concept Rehab is seeking a Certified Occupational Therapy Assistant (COTA/OTA) to join our team in Avon, Ohio, at a skilled nursing facility. Flexible Full-Time, Part-Time, and PRN schedules are available to meet stability and income needs.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active Ohio COTA license (or eligible).
• Strong communication and teamwork skills.
• Passion for working with geriatric and rehab populations.

Responsibilities

• Deliver occupational therapy treatments under the supervision of an OTR.
• Implement individualized care plans focused on ADLs and functional independence.
• Collaborate with therapists, nursing staff, and families.
• Maintain accurate and timely electronic documentation.
• Educate patients, caregivers, and staff on therapy programs.
• Assist with discharge planning and patient progress tracking.
