Job summary

Concept Rehab is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in a skilled nursing and post-acute care setting in Willoughby, OH. You will collaborate with Occupational Therapists and the interdisciplinary team to help patients regain independence and improve functional ability.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active Ohio COTA license in good standing.
• New graduates encouraged to apply.
• Experience in skilled nursing or post-acute care preferred.

Responsibilities

• Implement individualized treatment plans to improve functional performance.
• Provide skilled OT interventions to support independence in daily activities.
• Assist patients with ADLs such as self-care, dressing, grooming, bathing.
• Support upper extremity strength, fine motor coordination, balance, and mobility.
• Monitor progress and communicate changes to supervising therapists.
• Collaborate with nursing staff, rehab team, patients, and families.
• Educate caregivers and facility staff on therapeutic techniques and safety.
• Contribute to discharge planning and continuity of care.
• Maintain timely, accurate, and compliant electronic documentation.
• Promote a positive and engaging rehab experience for patients.
