Concept Rehab, Inc. seeks a Certified Occupational Therapy Assistant (COTA) to join our post-acute rehabilitation team in a Skilled Nursing Facility in Toledo, Ohio.

This position offers flexible scheduling as part-time and PRN opportunities and a supportive, therapist-led culture focused on helping patients regain independence. Under supervision of an OT, you will deliver treatments, assist with ADLs, and contribute to coordinated care planning within a multidisciplinary team, ensuring

Competenze

• Current Certified Occupational Therapy Assistant (COTA) license in Ohio.
• Graduate of an accredited COTA program.
• Strong communication and interpersonal skills.
• Commitment to compassionate, patient-centered care.
• Experience in Skilled Nursing, Long-Term Care, Rehabilitation, or Post-Acute Care preferred but not required.
• New graduates are encouraged to apply.

Mansioni

• Deliver occupational therapy treatments according to physician orders and established plans of care.
• Assist patients in improving performance with activities of daily living (ADLs).
• Support rehabilitation goals related to mobility, balance, strength, range of motion, cognition, safety awareness, and functional independence.
• Monitor and report patient progress, response to treatment, and changes in condition.
• Collaborate with Occupational Therapists, nursing staff, physicians, families, and interdisciplinary team members.
• Participate in care conferences, discharge planning, and treatment planning meetings.
• Complete accurate and timely documentation, treatment notes, and billing records.
• Educate patients, caregivers, and facility staff regarding therapeutic techniques, safety strategies, and restorative programs.
