Job summary

Concept Rehab, Inc. is seeking a Certified Occupational Therapy Assistant (COTA) to join our PRN team serving skilled nursing and rehabilitation facilities in the Dayton, OH area.

You will work with OTs and care teams to help residents regain independence through ADLs and functional mobility. Qualifications

• Current Ohio COTA license in good standing.
• Associate degree from an accredited OTA program.
• Strong communication and teamwork skills.
• Ability to complete documentation accurately.

Responsibilities

• Deliver skilled OT treatments under supervision.
• Assist patients with ADLs and functional mobility.
• Monitor, document, and communicate patient progress.
• Participate in interdisciplinary care planning and discharge planning.
• Maintain timely and accurate clinical documentation.
• Educate patients, families, caregivers, and facility staff.
• Recommend adaptive equipment and therapeutic interventions as appropriate.
• Support patient safety, independence, and quality of life.
• Provide compassionate, patient-centered care.
