Job summary

Personal Touch Home Care in Queens, NY is hiring a Certified Occupational Therapy Assistant (COTA) for a full-time position. You will provide compassionate OT services, help patients regain independence, and collaborate with a care team to deliver high-quality, personalized care.

This role supports pediatric and adult populations with flexible daytime and after-school cases, backed by an established home care agency with over 50 years of trusted service.

Qualifications

• Licensed/regulated as a Certified Occupational Therapy Assistant (COTA) in the practicing state, if required.
• Graduate of an ACOTE-accredited Occupational Therapy Assistant program.
• Strong verbal, written, and organizational skills.
• Pediatric experience or willingness to work with pediatric patients required.

Responsibilities

• Assess patient’s functional status and home environment; identify equipment needs and participate in the plan of care.
• Execute treatment program and goals for improved function; communicate plan of care to physician and organization professionals.
• May teach new skills or retrain patient in daily activities lost due to illness or injury.
• May fabricate splints and instruct patient in use of adaptive equipment to improve function.
• May train patient in prosthetic and/or orthotic device use.
• Maintain clinical records and reports; submit records per policy.
• Maintain contact with care team to promote coordinated care.
• Attend in-services, case conferences, and meetings for coordinated plans of care.
• Identify patient and family needs for other home health services; refer as appropriate.
• Instruct family/caregiver and other health personnel in treatment regime.
• Participate in discharge planning for patient.
