Job summary

Frontline Personnel Services is seeking a Social Worker to join our team in New York. You will assess, counsel, interview, and educate clients and their families, and help plan care in line with facility policies to meet emotional and social needs.

Full-time position with benefits including health, dental, vision, 401k, and daily pay options. Experience in nursing home or skilled nursing facility is required; strong communication and teamwork are essential.

Qualifications

• Nursing home or skilled nursing facility experience required

Responsibilities

• Assess patient situations by interviewing and reviewing history.
• Plan and implement support packages to help patients and families.
• Develop and apply treatment plans, including Medicaid/Medicare applications.
• Assist with Food Stamps, housing applications and phone access.
• Document patient progress and maintain accurate records.
• Promote patient rights and ensure a safe, dignified environment.
• Coordinate referrals to other healthcare professionals.
