Qualifications

• Minimum 1 year of CNA experience in SKN settings.
• Active OH CNA certification required.
• High school diploma or equivalent.
• Reliable transportation to facilities.
• Consistent attendance and punctuality.
• Legally allowed to work in the United States.
• Willing to undergo background and drug screening.

Responsibilities

• Record patient data and vital signs in the EMR.
• Assist residents with activities of daily living.
• Provide safe transfers and mobility assistance.
• Document vital signs and observations as assigned.
• Respond promptly to resident call lights.
• Report changes to the nursing team.
• Assist with meals and hydration as needed.
• Maintain infection control and a safe environment.
• Support nursing staff in resident-centered care.
• Communicate concerns to supervisor and agency.
• Other duties as assigned.
