Qualifications

Minimum 1 year of experience working under an active CNA Certification.
Minimum 1 year of experience working in skilled nursing.
Must have reliable transportation to assigned facilities.
Must maintain consistent attendance and punctuality.

Responsibilities

Utilize EMR to record patients’ medical information and vital signs.
Assist residents with activities of daily living including bathing, dressing, grooming, and toileting.
Provide safe transfers, repositioning, and mobility assistance using proper body mechanics.
Obtain and document vital signs and other resident observations as assigned.
Respond promptly to resident call lights and requests for assistance.
Report changes in resident condition, behavior, or concerns to the nursing team.
Assist residents with meals, hydration, and feeding when needed.
Maintain a clean, safe, and respectful environment following infection control practices.
Support the nursing team in delivering quality resident-centered care.
Communicate concerns or issues to onsite supervisor and agency.
