Qualifications

• 1 year CNA experience in a skilled nursing setting
• Active OH CNA certification required
• Reliable transportation and punctuality
• Ability to lift up to 50 pounds and assist with transfers

Responsibilities

• Record patients’ medical information and vital signs
• Assist residents with activities of daily living (bathing, dressing, grooming, toileting)
• Provide safe transfers and mobility assistance with proper body mechanics
• Document vital signs and observations as assigned
• Respond to resident call lights promptly and request for assistance
• Report changes in condition to the nursing team
• Assist with meals, hydration, and feeding as needed
• Maintain a clean, safe, infection-controlled environment
• Support the nursing team in delivering resident-centered care
• Communicate concerns to onsite supervisor and agency
• Perform other duties as assigned

Education
