Job summary

Midtown Oaks Post Acute in Sacramento, CA is seeking caring CNAs to provide routine daily nursing care per care plans under licensed staff. The role requires an active CNA certification and may include CPR credentials; strong communication and teamwork are essential.

Qualifications

• Active CNA certification is required.
• CPR certification is preferred.

Responsibilities

• Assist residents with activities of daily living (bathing, dressing, grooming, toileting).
• Take and record vital signs and document findings.
• Monitor resident condition and report changes promptly.
• Assist with transfers, repositioning, and mobility support.
• Answer call lights and ensure resident comfort and safety.
• Provide mealtime support, including feeding and documenting intake.
• Maintain clean, organized, and safe resident rooms.
• Document care accurately in charts and flow sheets.
• Observe and report skin issues, injuries, or changes in behavior.
• Support admissions, transfers, and discharges.
• Follow infection control and HIPAA guidelines.
