Job summary

Georgia HOPE is seeking a Client Support Specialist to deliver community-based support to individuals and families in their homes and local environments. You will help clients build skills, access resources, and move toward greater independence and stability, working closely with therapists and care teams.

The role emphasizes relationship-driven, skill-focused support, with responsibilities including coordinating with families, monitoring progress, and documenting in the EMR system.

Qualifications

• High School Diploma with at least two years of relevant experience, or a Bachelor’s degree in a behavioral health or related field.
• Experience working with children, adolescents, or adults with mental health or behavioral health needs preferred
• Valid driver’s license and reliable transportation for community-based work

Responsibilities

• Deliver community-based support services in homes and other real-world settings.
• Maintain regular contact with assigned clients, including in-person and phone-based support.
• Review client history, assessments, and treatment plans to ensure services align with goals.
• Support clients and families in developing and achieving goals in their IRP.
• Teach coping strategies, emotional regulation, and daily living skills.
• Help clients identify triggers, build resilience, and develop appropriate behavioral responses.
• Assist with school, work, and family functioning by reinforcing practical, real-life skills.
• Connect clients and families to community resources that support basic needs and long-term success.
• Proactively support crisis prevention by helping clients develop safety plans.
• Collaborate with therapists and clinical teams for coordinated care.
• Track service utilization and document in EMR.
