Job summary

Georgia HOPE, part of the Health Connect America family, is seeking a Client Support Specialist to provide community-based support to individuals and families. You’ll help build skills, access resources, and move toward independence in clients’ everyday environments.

You’ll work closely with therapists and teams to ensure coordinated, whole-person care, documenting progress and adapting plans as needed. A valid driver’s license is required for community-based work.

Qualifications

• High School Diploma or higher in behavioral health or related field.
• Experience with children, adolescents, or adults with mental health or behavioral health needs preferred.
• Valid driver’s license and reliable transportation for community-based work.

Responsibilities

• Provide hands-on, community-based support in homes and real-world settings, meeting clients where they are.
• Maintain regular contact with assigned clients, including in-person and phone-based support.
• Review client history, assessments, and treatment plans to align with goals.
• Support clients and families in developing and achieving goals in their IRP.
• Teach coping strategies, emotional regulation, and daily living skills.
• Help clients identify triggers, build resilience, and develop appropriate behavioral responses.
• Assist with school, work, and family functioning by reinforcing real-life skills.
• Connect clients and families to community resources for basic needs and stability.
• Collaborate with therapists and teams to ensure coordinated care.
• Track service utilization and document in the EMR system.

Education

High School Diploma or higher in behavioral health or related field

Bachelor’s degree in behavioral health or related field
