Qualifications

• Must be a graduate of an accredited or state-approved RN program.
• Current Massachusetts RN license.
• Two years of clinical nursing experience.
• Strong clinical assessment, communication, and organizational skills.

Responsibilities

• Complete comprehensive patient admission assessments and documentation.
• Perform medication reconciliation and identify potential medication-related concerns.
• Coordinate and facilitate timely patient admissions and discharges.
• Provide education to patients and families regarding hospitalization and discharge instructions.
• Collaborate with physicians, case managers, nursing staff, and ancillary departments to support care transitions.
• Advance patient throughput initiatives and care coordination.
