Job summary

Beth Israel Lahey Health is seeking a Registered Nurse to join the Patient Flow, Admissions & Discharge team at Winchester Hospital. The role focuses on coordinating admissions and discharges across hospital areas, enabling bedside nurses to spend more time on direct patient care while improving throughput and patient outcomes.

We welcome candidates with strong assessment and communication skills, a current MA RN license, and at least two years of clinical nursing experience.

Qualifications

• Graduate of an accredited or state-approved Registered Nursing program.
• Current Massachusetts RN license.
• Two years of clinical nursing experience.
• Strong clinical assessment, communication, and organizational skills.
• BSN strongly preferred.
• Experience in Medical-Surgical Nursing.
• Experience in Care Coordination/Transitions of Care.
• Experience in Patient Flow or Throughput Operations.

Responsibilities

• Complete comprehensive patient admission assessments and documentation.
• Perform medication reconciliation and identify potential medication-related concerns.
• Coordinate and facilitate timely patient admissions and discharges.
• Provide individualized patient and family education regarding hospitalization, care plans, and discharge instructions.
• Collaborate with physicians, case managers, nursing staff, and ancillary departments to support effective care transitions.
• Advance patient throughput initiatives and care coordination efforts.
• Help reduce administrative demands on bedside nursing teams, creating greater capacity for direct patient care.
• Promote a culture of safety, efficiency, and patient-centered excellence throughout the hospitalization journey.
