Nutrition that Works, LLC is seeking a consultant dietitian to join our team in Welch, WV, for 24 hours per week. This role offers a flexible schedule and competitive pay within a supportive long-term care setting.

You will conduct clinical nutrition assessments, document using the Nutrition Care Process in the EMR, and collaborate with the interdisciplinary care plan team. Responsibilities include completing MDS - Section K, calculating TF, and recommending care changes for wounds and weight

Formación

• CDR credentialed dietitian with current licensure.
• Licensed in nutrition and dietetics practice.
• Positive attitude and client-centered approach.
• Excellent communication with interdisciplinary teams.
• Experience in long-term care is a plus.

Responsabilidades

• Perform clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
