Job summary

Must be a graduate of an APTA-accredited PT program with a valid state license and strong documentation skills.

Qualifications

• Graduate of an APTA accredited school of physical therapy.
• Valid Physical Therapy state license.
• Must be within the 50 miles radius.
• Proficient in clinical documentation.
• Basic Internet navigation skills.

Responsibilities

• Provide physical therapy and related services to patients.
• Develop initial and ongoing plans of care; supervise assistants and Rehab Techs.
• Manage discharge planning and case management responsibilities.
• Maintain professional conduct in line with quality and service standards.
