Job summary

Talented Medical Solutions is seeking a licensed Physical Therapist to provide PT services for patients, following Reliant Rehabilitation guidelines. You will screen, evaluate, and develop initial and ongoing plans of care, manage discharge planning, and supervise Rehab Techs and assistants, while upholding professional conduct.

Must be within 50 miles of the facility. Requires graduation from an APTA-accredited program and a valid state license; computer-literate with clinical documentation

Qualifications

• Must be within the 50 miles radius.
• Education/Experience: A graduate of an APTA accredited school of physical therapy.
• Computer Skills: Computer literate and proficient in clinical documentation
• Basic Internet navigation skills
• Certificates and Licenses: Valid Physical Therapy state license

Responsibilities

• Provide physical therapy and related services to patients.
• Develop initial and ongoing treatment plans of care.
• Discharge planning and supervision of assistants and Rehab Techs.
• Engage in case management and maintain professional conduct per regulations.
