First Home Healthcare LLC is seeking a PHC Field Supervisor to join a growing team in the San Antonio area. The ideal candidate will be a reliable team player with strong communication skills and organizational abilities, capable of recruiting and supervising attendants, and ensuring high-quality home care services.

The role involves performing initial home visits to establish care plans, maintaining client charts, and ensuring compliance with applicable regulations.

Qualifications

• High School diploma or equivalent.
• At least 2 years experience as a PHC Supervisor.
• Proficient with Vesta software.
• Bilingual in English and Spanish.

Responsabilités

• Assess client’s functional status and take necessary action to ensure client’s well‑being.
• Promote, review, and evaluate the quality and appropriateness of client care practices.
• Ensure services are delivered in full compliance with DADS/Contractor/HMO regulations, contract standards, and agency policy.
• Recruit, interview, hire, train, supervise, evaluate, and terminate attendants as needed.
• Make initial home visits to develop a plan of care, orientation, and attendant hiring.
• Complete paperwork in client charts and copy to clients.
• Assign attendants to clients based on client needs.
• Review and evaluate attendants’ performance.
• Support office functions and processes including clerical, personnel, and medical records.
• Stay current with provider services regulations.
• Perform other duties as assigned.
