Qualifications

• Current, unrestricted Social Worker license or LMHC in Hawaii
• 2+ years of social work or similar experience
• Experience with long-term care, home health, hospice, or assisted living
• MS Word, Excel and Outlook proficiency
• Willingness to travel within assigned region to visit Medicaid members
• Access to internet via landline or mobile network
• Driver's license and reliable transportation

Responsibilities

• Assess, plan and implement care strategies individualized for each patient
• Identify and initiate referrals for social services
• Manage the care plan as a single point of contact
• Communicate required health information to stakeholders for coordinated care
• Advocate for patients and families to ensure needs are met by the health care team
