Job summary

UnitedHealthcare is seeking an RN Health and Social Services Care Coordinator to coordinate long-term care needs for patients and advocate for the best possible care within an Integrated Care Model. The role supports a field-based team with remote options and local travel on Oahu, HI.

You will assess care plans, coordinate referrals to social services, and serve as the single point of contact across the continuum of care, collaborating with diverse health care professionals to improve outcomes

Qualifications

• Current, unrestricted RN license in Hawaii.
• 2+ years of clinical experience.
• Experience with long-term care, home health, hospice, or assisted living.
• Intermediate level of proficiency with MS Word, Excel and Outlook.
• Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings.
• Driver's license and access to reliable transportation.
• Current access or ability to obtain internet access via a landline.

Responsibilities

• Assess, plan and implement care strategies individualized by patient.
• Identify and initiate referrals for social service programs.
• Manage the care plan throughout the continuum of care as a single point of contact.
• Communicate with stakeholders the required health-related information to ensure quality coordinated care.
• Advocate for patients and families to ensure needs and choices are represented.
