Preventive Measures Home Health Care (PMHHC) is seeking a Field Service Supervisor to oversee Direct Care Workers and ensure high-quality, compliant home health services in Allentown, PA. You will guide DCWs on the Plan of Care, monitor EVV clock-ins, and support client wellness and referrals.

Responsibilities

• Fill and cover shifts and manage coverage as needed.
• Track overtime and coordinate with Payroll for approvals.
• Oversee referrals and onboarding of potential clients.
• Manage on-call phone coverage and schedules.
• Assist intake with new clients and link patients to community resources.

Preventive Measures Home Health Care (PMHHC) provides support for people from all walks of life to live independently at home and be part of the community.

Field Service Supervisor ( FSS ) is responsible for facilitating and overseeing the Direct Care Workers (DCW) who provide direct care services for our clients. FSS ensured that DCWs are trained on the client P lan of Care (POC) providing high quality and efficient health care and supportive services . FSS will provide self-management goals and referrals to both internal and external resources. Responsible for the training, supporting and the development of all FSS (s) . The Field Supervisor is responsible to ensure that they review the EVV of DCWs to ensure that the tasks are complete and to monitor the clock in and clocking out of shifts.

Primary Job Responsibilities:

• Assist with monthlyClient QA Visitsand completion of Client Survey on caseload

• Client Wellness Check-ups– The clients assignedDFOwill organize all WellnessCheck-upsvisits. The Director may also requestFSSs to conductWellness Check-upsvisits
