Growing Home Care Agency in Brooklyn, NY is seeking a Field RN Case Manager to assess clients, create individualized care plans, and conduct regular home visits. The role includes guiding Home Health Aides and Personal Care Assistants to ensure high-quality care.

Responsibilities

• Provide in-service trainings and guidance to Home Health Aides and Personal Care Assistants.
• Collaborate with the interdisciplinary team to ensure proper patient care.
• Provide education and emotional support to clients and families.
• Maintain accurate and up-to-date patient records and documentation.
• Participate in team meetings and contribute to care strategy development.
