Job summary

BayCare HomeCare in Largo, FL is seeking a Home Health RN to deliver hands-on care in patients' homes across assigned areas including Largo, St. Petersburg and surrounding neighborhoods. The position runs 8:00 AM–5:00 PM with some weekend work and on-call duties.

Qualifications

• RN must hold a valid Florida RN license.
• BLS certification required.
• Driver's license in Florida within 30 days of hire.
• Education: Associate's or Diploma in Nursing; BSN preferred.
• Minimum 1 year of RN experience; home health preferred.

Responsibilities

• Oversee the activities of the multi-disciplinary team that provides care to the patient.
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
• Ensure that all visits scheduled and provided are authorized and covered by physician orders.
• Review patient visits weekly and provide plan to Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Inform the scheduler of schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

• Home visits in assigned service area: Largo, St. Petersburg, Pinellas Park surrounding areas assigned by zip code.

The Home Health Field, RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

Home Health RN Requirements:

• BLS (Basic Life Support)

• RN (Registered Nurse) License

• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

• Required Associate's or Diploma Nursing

• Required 1 year Registered Nurse

• Preferred Bachelor's Nursing

• Preferred 1 year Home Care
