Job summary

MedStar Health is seeking a Field RN Case Manager to oversee comprehensive care coordination for DC enrollees, integrating physical and behavioral health, social and environmental factors into a unified plan.

This role requires up to 85% travel within the DC region, collaboration with providers and community resources, and responsibility for utilization management, referrals, and ensuring person-centered service planning.

Qualifications

• Bachelor's degree in Nursing is required.
• 1-2 years experience in social service or health care related setting.
• 1-2 years experience with Developmental Disabilities HIV Behavioral Health and/or other vulnerable populations.

Responsibilities

• Integrates physical health, behavioral health, social and environmental factors into a single person-centered system of care.
• Promotes person-centered service planning that honors Member priorities and preferences.
• Encourages enrollee engagement in service planning with teams skilled in community integration and health/wellness.
• Leverages providers, community partners and local resources for comprehensive, coordinated and culturally appropriate care.
• Provides service and support to maintain members in the least restrictive setting.
• Coordinates care with evidence-based guidelines and decision support tools.
• Provides Case Management for designated member population, including self-direction and transition of care.
• Oversees daily/weekly/monthly functions of Field Case Manager and prioritizes needs for clinical reviews.
• Prepares reports to managers on work in progress and backlogs.
• Informs members about annual assessments, service options and rights including Fair Hearing.
