Candidates must be a Texas-licensed RN with at least 1 year of Medicare home health experience, a valid Texas driver’s license, CPR certification, and willingness to travel to multiple locations.

Qualifications

• Licensed Registered Nurse in Texas.
• Minimum 1 year Medicare home health experience.
• Valid Texas driver's license and auto insurance.
• Willingness to travel to multiple locations.
• CPR certification.
• Knowledge of WellSky EMR software desired.

Responsibilities

• Provides clinical care including wound care, medication administration, disease process education and IV therapy.
• Ensures clinical documentation demonstrates medical necessity and reflects quality patient care.
• Submits OASIS and other clinical documentation in a timely manner.
• Communicates patient's clinical needs with interdisciplinary team.
