Job summary

21st Century Home Health Services is seeking an entry-level Field Registered Nurse to develop fundamental nursing skills in a home health setting. You will work under a Case Manager RN, visit patients in their homes, and learn to align care with the company’s operational goals and efficiencies.

The role emphasizes hands-on patient care, documentation within 24–48 hours, and collaboration with the interdisciplinary team.

Qualifications

• One year of recent nursing experience; home health experience preferred.
• Graduate of an accredited school of nursing and CA RN license.
• Current BLS certificate and valid CA driver’s license.

Responsibilities

• Provide hands-on patient care within RN scope in home settings.
• Implement and document plan of care with Case Manager RN.
• Coordinate services with interdisciplinary team and report findings.
• Maintain timely documentation within 24–48 hours and ensure privacy.
