Job summary

Zaddy Solutions is seeking a Home Health RN Case Manager for a field-based role serving the Dallas/Fort Worth area. The role requires recent home health case management experience, strong assessment and documentation skills, and the ability to manage a caseload independently in the field.

The position covers Dallas area south of Downtown and surrounding counties, with PRN scheduling and local travel throughout Fort Worth / Tarrant County. A Texas RN license in good standing is required.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver’s license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.

Territory: Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.
