Job summary

CVS Health is seeking a Case Manager, Registered Nurse Field position in Des Plaines, IL to assess, plan, implement, and coordinate care for members with complex health and social needs, including dual eligible populations.

The role requires an IL RN license with 3–5 years of clinical experience, and preferred experience in care management or discharge planning. Travel is 50–75% across Dupage/Cook Counties, with home visits and telephonic interactions.

Qualifications

• Associate's degree required, Bachelor's preferred
• Minimum 3-5 years clinical practical experience
• Current RN License in Illinois required

Responsibilities

• Develop proactive plans of care to enhance member outcomes.
• Conduct evaluations of member needs using clinical tools and data review.
• Apply clinical judgment to reduce risk factors and address complex health indicators.
• Conduct assessments considering various information sources, including claims.
• Use a holistic approach to determine the need for referrals to clinical resources.
• Collaborate with supervisors and stakeholders to overcome barriers in care.
• Utilize case management processes in compliance with regulatory policies.
• Employ motivational interviewing skills to engage members effectively.
• Interact with members telephonically or in person, including home visits.
• Present cases at interdisciplinary case conferences.
